Tacrolimus partitions extensively into red blood cells, so whole blood measurement is the appropriate sample type. Therapeutic ranges vary depending on the type and age of transplant, indication, and concomitant immunosuppression.
This test is used for routine monitoring in transplant patients, evaluation of suspected toxicity or rejection, after medication changes, and when drug interactions may alter tacrolimus levels. Trough levels (just before the next dose) are standard. Sample type is whole blood in an EDTA tube.
