Tacrolimus has a very narrow therapeutic range — too little allows organ rejection, while too much can cause kidney toxicity, neurological side effects, and infections. Regular blood level monitoring is required throughout treatment.
This test is ordered routinely in transplant patients (initially frequent, then less frequent once stable), to evaluate suspected toxicity or rejection, after medication changes, and when interacting drugs are added or removed. Trough levels — drawn just before the next dose — are the standard for monitoring. Sample type is whole blood in an EDTA tube.
