Pheochromocytomas and paragangliomas can produce excessive catecholamines that cause severe hypertension, headache, sweating, and palpitations. Catecholamines are rapidly broken down to metanephrine and normetanephrine, which are more stable and reliably detectable. Fractionated plasma free metanephrines are highly sensitive for detecting these tumors.

This test is used in patients with episodic symptoms suggesting catecholamine excess, severe or resistant hypertension, incidentally found adrenal masses, and family history of pheochromocytoma or related genetic syndromes. Sample preparation requires the patient to be supine for at least 30 minutes before collection. Sample type is plasma.
