Most of the body's magnesium is inside cells, with only about 1% in serum. Serum magnesium can remain in the normal range even when intracellular stores are depleted, so RBC magnesium provides a more sensitive assessment of magnesium status.
This test is used when magnesium deficiency is suspected despite normal serum magnesium, in evaluation of unexplained muscle cramps, fatigue, arrhythmias, or refractory hypokalemia, and in patients with conditions causing chronic magnesium loss. Sample type is whole blood in an EDTA tube.
