LACTATE DEHYDROGENASE (LD) ISOENZYMES

LD exists as five isoenzymes (LD1-LD5), each predominating in different tissues: LD1 and LD2 in heart and red cells, LD3 in lungs and spleen, LD4 and LD5 in liver and skeletal muscle. The pattern of elevation among isoenzymes helps identify the tissue source.

This test is used when total LD is elevated and the source is unclear, in evaluation of unusual hemolysis, certain cancers (especially germ cell tumors), and tissue injury patterns. Modern testing increasingly relies on tissue-specific markers (such as troponin for cardiac injury), but LD isoenzymes still have selected uses. Sample type is serum.
