Anti-HBc total includes both IgM (recent infection) and IgG (past infection) antibodies. Unlike hepatitis B vaccination, which produces only surface antibody, hepatitis B infection produces core antibody. So anti-HBc total positivity indicates either past or current infection — but not vaccination.
This test is used in hepatitis B screening (to distinguish past infection from vaccination), pre-transplant evaluation, immunosuppression workup (to identify patients at risk of reactivation), and blood donor screening. Results are typically interpreted alongside HBsAg and hepatitis B surface antibody. Sample type is serum.
