Anti-HBc total includes both IgM (recent infection) and IgG (past infection) antibodies. Unlike hepatitis B vaccination, which produces only surface antibody, hepatitis B infection produces core antibody — so a positive anti-HBc total indicates past or current infection, not vaccine response.
This test is used in hepatitis B screening to distinguish past infection from vaccination, pre-transplant evaluation, immunosuppression workup (to identify patients at risk for HBV reactivation), and blood donor screening. Results are interpreted alongside HBsAg and hepatitis B surface antibody. Sample type is serum.
