Red cell folate reflects folate stores accumulated over the lifespan of red blood cells (about 120 days), making it more representative of long-term folate status than serum folate, which can fluctuate with recent intake. It is particularly useful when serum folate may not accurately reflect tissue stores.
This test is used when chronic folate status needs assessment, in evaluation of anemia or other deficiency-related findings, and when serum folate results are equivocal. It is also helpful in monitoring patients on long-term folate-affecting medications. Sample type is whole blood in an EDTA tube.
