Standard lipid panels calculate LDL from total cholesterol, HDL, and triglycerides using the Friedewald formula, which becomes inaccurate when triglycerides exceed 400 mg/dL. Direct LDL measurement is unaffected by triglyceride levels and remains accurate in non-fasting samples.
This test is used in patients with high triglycerides where standard LDL calculation is unreliable, in non-fasting samples, and when more precise LDL measurement is needed for treatment decisions. It is often added when standard lipid panel results need clarification. Sample type is serum.
