Carbamazepine has a narrow therapeutic range, meaning the difference between an effective dose and a toxic one can be small. Blood level monitoring is therefore important for safe and effective treatment.
This test is ordered to confirm that levels are within the therapeutic range, evaluate suspected toxicity (which can present as drowsiness, confusion, ataxia, or arrhythmias), assess treatment adherence, and adjust dosing when starting therapy or changing other medications. Trough levels — drawn just before the next scheduled dose — are typically most useful. Sample type is serum.
