C. difficile causes disease through production of toxin A and toxin B. The enzyme immunoassay (EIA) detects these toxins directly in stool, providing evidence of active toxin-producing infection rather than mere colonization. However, EIA testing is less sensitive than molecular (PCR) testing for the toxin gene.
This test is used in evaluation of significant diarrhea, particularly with recent antibiotic exposure or hospitalization. Many labs now use a multi-step algorithm combining different testing methods to maximize diagnostic accuracy. Sample type is unformed stool.
